Why mobile documentation
Paper isn't the problem. Delay is.
Providers still using paper Leistungsnachweise don't suffer from paper. They suffer from the slip arriving at the office days later, getting typed up, then lying around for weeks until the end-of-month Pflegekasse billing. Disputes arise in that gap — and they always come out to the provider's disadvantage.

Voice to note
Daniel dictates an observation as he leaves the apartment. Aldor transcribes, attaches the note to the visit, files it. Lotte summarises the last ten notes for a client when someone new takes over.
Complete history
Every change — corrected timestamp, retroactively-added service — with person, time, previous value. At the audit you don't have to explain anything that isn't in the log already.
I let the old system run in parallel for six weeks. After six weeks, it was clear I wasn't turning Aldor off.
